Clinical support after discharge

Medical Care at Home

Some patients need additional support after hospital discharge or while recovering in a hotel, residence or short-term accommodation. When medically appropriate, we can coordinate home healthcare through licensed nurses and doctors, including nursing support or a doctor visit. The treating medical team must first confirm that discharge and out-of-hospital care are safe. If this service is recommended, your coordinator will clarify the provider, schedule, scope of care and escalation plan before support begins.
